Job Description

Locum Community Nurse (Band 5)

Location: Shrewsbury and Telford, Shropshire
Contract: Locum – ASAP start, 12 weeks
Hours: Full-time – 09:00-17:00 (Part-time considered, 3-4 days/week)
Specialty: Home Treatment Team (HTT)

Mind Professionals is looking for 2 Registered Mental Health Nurses with knowledge of Dementia for full-time locum work as Community Nurses working at a Band 5 grade.

The post holders would join a Home Treatment Team (HTT), working in Shropshire from two sites, one in Shrewsbury and the other in Telford.

Key Responsibilities:

– Take responsibility with supervision and support from a Band 6 Nurse, for the management of an identified caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans
– Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
– Assess patient’s individual care needs, develop, implement, and evaluate programmes of care.
– Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions.
– Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
– Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
– Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.

Requirements:

– NMC registration
– Experience working in a Home Treatment Team (HTT) (desirable)
– Knowledge of Dementia
– Driving licence and access to a vehicle
