Locum Community Nurse (Band 6)
Location: Oswestry and Market Drayton, Shropshire
Contract: Locum – ASAP start, 8 weeks
Hours: Full-time (37.5 hrs/week) – 09:00-17:00
Specialty: Adult / Community (CMHT)
Mind Professionals are looking for a Community Nurse for full-time locum work as part of an Adult Community pathway service, working across Oswestry and Market Drayton, Shropshire.
Key Responsibilities:
– The post holder will be responsible for working in an identified adult community mental health team, with the flexibility of working across localities. They will be responsible for care coordination of an identified caseload.
– The post holder will be responsible for undertaking clinical assessments to establish the person’s needs, goals, risks and strengths to generate a collaborative care plan agreed with all involved.
– The post holder will be responsible for delivering recovery focused interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.
– The post holder will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.
Requirements:
– NMC registration
– Experience working in an Adults functional SMI service
– Driving licence and access to a vehicle, with business insurance
