Job Description
Physiotherapist Specialist
Grade/ Band: 6
Location: Noth East London
Contract: Locum
Hours: Full time
Specialty: Adult Intensive Rehab and Community Treatment
Key responsibilities:
– Provide urgent physiotherapy assessment and treatment to prevent unnecessary hospital admission. Respond rapidly to referrals for patients experiencing acute functional or mobility decline.
– Assess mobility, balance, strength, transfers and falls risk within community settings.
– Deliver timely rehabilitation interventions to maximise independence and reduce deterioration.
– Develop and implement immediate management plans to support patients safely at home.
– Work collaboratively with multidisciplinary teams to provide coordinated crisis response.
– Facilitate early supported discharge and prevent hospital readmission where appropriate.
– Recommend equipment, exercise programmes and environmental adaptations to reduce risk.
– Support patients and carers with education, self-management strategies and rehabilitation goals. Maintain accurate clinical documentation and contribute to service performance outcomes.
Requirements:
– Experience in NHS and post qualification as an PT, preferably within the community and rapid response setting.
– Full UK driving license required given the community role across 3 London boroughs
